Asbestos Register – (Originators Correction)

Some years ago when I was serving in OTAGO, they found a fault in the steam pipes which necessitated us going alongside Pearl Harbour to have them replaced and relagged. At the time we were all advised to register with the Asbestos Registration, which I did. I have since spoken with a number of ex sailors and enquired as to whether they had registered having served on ships with asbestos in the pipe lagging. Some had and some had not. So here is the form required to register for exposure to asbestos.
